Align Health Co is committed to protecting your personal information and health information in accordance with the Privacy Act 1988 (Cth) and the Australian Privacy Principles (APPs).
As a health service provider, we handle sensitive health information and take our obligations under the Privacy Act seriously. This policy explains what information we collect, how we use it, who we share it with, and how you can access or correct it.
What Information We Collect
Personal Information Name, date of birth, contact details (address, phone number, email), emergency contact details, and health fund membership details.
Health Information Medical history, current and past conditions, medications, clinical notes, treatment records, imaging results, and any other information relevant to providing chiropractic care. Health information is treated as sensitive information under the Privacy Act and is subject to additional protections.
Payment Information Payment method details and transaction records. We do not store full credit card numbers.
Website Usage Data We use Google Analytics 4 (GA4) to collect anonymised data about how visitors use our website. This includes pages visited, time on site, and device type. This data does not identify individuals.
How We Collect Information
Most information is collected directly from you at the time of your appointment, through our intake forms, or during phone or email contact.
We may also receive information from other health practitioners where you have consented to a referral or where your care requires coordination with another provider.

How We Use Your Information
We use your information to:

Who We Share Your Information With
Within the Clinic Your information is accessible to the treating practitioners and administrative staff at Align Health Co for the purposes of providing your care.
Other Health Practitioners We may share relevant information with other health practitioners (GPs, specialists, or allied health professionals) where this is necessary for your care and where you have consented.
Third-Party Service Providers We use third-party software for booking management, practice management, and payment processing, including Cliniko for practice management and secure electronic record storage. These providers are bound by their own privacy policies and by contractual obligations to handle your data appropriately.
Regulatory Bodies We may be required to disclose information to regulatory bodies (AHPRA, Medicare, health funds) as required by law.
Emergency Situations In an emergency where there is a serious threat to your life or safety, or the life or safety of another person, we may disclose relevant information to emergency services or treating practitioners without your prior consent.
Security
Your records are stored electronically in Cliniko, with access restricted to authorised clinic staff. Electronic records are protected by password and two-factor authentication.
In the event of a data breach that poses a real risk of harm, we will notify affected individuals and the Office of the Australian Information Commissioner (OAIC) as required under the Notifiable Data Breaches scheme.
Accessing and Correcting Your Information
You have the right to request access to the personal and health information we hold about you. Requests should be made in writing to hello@alignhealthco.com.au.
We will respond within 30 days. In most cases, access will be provided free of charge. If a request is complex or requires significant effort to fulfil, a reasonable fee may apply.
If you believe information we hold is inaccurate, incomplete, or out of date, you can request a correction. We will update the record or, where we disagree with the requested correction, note your concern in the record.
Retention of Records
Health records are retained for the period required by Queensland Health legislation — a minimum of seven years from the date of last contact, or until the patient is 25 years of age, whichever is the later.
Complaints
If you have a concern about how we have handled your personal or health information, contact us at hello@alignhealthco.com.au. We will acknowledge your complaint within five business days and aim to resolve it within 30 days.
If you are not satisfied with our response, you can lodge a complaint with the Office of the Australian Information Commissioner (OAIC) at oaic.gov.au or by calling 1300 363 992.
